Establishing Care and Screening Process
The provider will set up a phone call to screen patients. This is to ensure that our clinic is a good fit for your mental health needs and vice versa. The initial appointment will then be confirmed at this time.

No Controlled Substances
The provider DOES NOT PRESCRIBE ANY CONTROLLED SUBSTANCES. These include stimulants (Adderall/mixed amphetamine salts, Vyvanse/lisdexamfetamine, Ritalin/methylphenidate, Dexedrine/dextroamphetamine SR), Concerta/methylphenidate SR, etc.) and benzodiazepines (Xanax/alprazolam, Klonopin/clonazepam, Ativan/lorazepam, Valium/diazepam).

No disability claims, work accommodations, emotional support animal letters, and/or workman’s compensation
The provider does not support disability claims, work accommodations, emotional support animal letters, and/or workman’s compensation claims. All other letters are subject to the provider’s discretion and are NOT a guarantee. Patients must be established for AT LEAST 6 VISITS if such a request for a letter is granted.

Appointments and Cancellations
Appointments are scheduled in advance based on mutual availability. Payments for each appointment will be made through Headway by debit or credit card or ACH transfer. You may cancel appointments in advance without charge, as long as you give at least 24 hours notice. For appointment no-shows or last-minute cancellations LESS THAN 24 HOURS, you will be charged a fee of $75.

Availability and After-Hours Emergencies
Providers check for voicemail messages during normal business hours. Messages left outside of normal hours of operation will be picked up the next business day. If you are experiencing suicidal or homicidal thoughts, are in crisis, or need immediate help, please call 911 or go to the nearest emergency department. The provider is often not immediately available by telephone especially when in a session with clients. At these times, you may leave a message at the confidential voicemail and your call will be returned, however, it may take 24-48 hours for non-urgent matters.

Discharge Process
There are several reasons why the clinic may eventually end a professional relationship. You may decide you would prefer to work with a different provider or the provider would come to the conclusion that you would be better served working with another professional. Regardless of the case, the provider will discuss the reasons for discharging, and if you request, provide you with a list of other qualified providers.

Please note that ongoing failure to pay for treatment, attend sessions, or communicate with the provider in a respectful and timely manner can also result in discharge from the practice. In these instances, to ensure you have continued access to care, the provider will still make every reasonable effort to get in touch with you and provide referrals to a new provider before the provider considers the relationship has ended.
